Concast Birmingham, Inc. dba Concast Metal Products located in Wakeman, OH: On or about March 17, 2020, the employer did not ensure the written lead compliance program contained the following information: 1. A description of each operation in which lead is emitted; e.g. machinery used, material processed, controls in place, crew size, employee job responsibilities, operating procedures and maintenance practices; 2. A description of the engineering controls in place to reduce employee exposures to lead; 3. A description of the administrative controls such as employee rotation, used to reduce employee exposures.
