29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Yates 2 1/4 Side-Matcher; On or about January 14, 2020 and at times prior thereto, the employer exposed employees to struck-by, caught-in, and amputation hazards in that the employer failed to ensure energy control procedures were utilized when employees were performing servicing and maintenance activities, such as jam clearing, on the Side Matcher.
