29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a)Grangaard Construction, Inc. at the Highway 212 Bridge, Watertown, SD 57201: On or about and at times prior to October 19, 2019, the exposing employer did not ensure that employees were protected from falls while walking on temporary elevated wooden platforms to access formwork on a bridge pier within a cofferdam. The platform was not equipped with guardrails nor did employees have the ability to use a personal fall arrest system on the platforms. On October 19, 2019, an employee fell off of an elevated platform and fell approximately 20 feet to the concrete footer below the pier. The injured caused the to a fall hazard of approximately 20-foot and resulted in an employee falling from the platform and being seriously injured. (b)Grangaard Construction, Inc. at the Highway 212 Bridge Project, Watertown, SD 57201: On or about and at times prior to October 19, 2019, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet or more above a lower level was protected from falling. Employees were walking and working from the approximately 24-inch wide I-beams within the cofferdams with no fall protection system in place. This condition exposed employees to a fall hazard of approximately 20 feet.
