29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. On or about December 11, 2019 the employer failed to ensure that work related injuries which occurred at the facility were properly documented using OSHA 300, 301, and 300A forms or their equivalent. A workplace injury occurred on December 3, 2019. The employer had not recorded the injury or maintained injury logs for previous calendar years.
