On November 16, 2019 at 220 Industrial Avenue in Clintonville, employer did not ensure written specific Lockout Tag Out procedures had been developed prior to the operation of the T-Nut Pneumatic/Electric Power Press #330; the press inserts four T-Nuts into plastic chair-liner parts.
