29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. On or about November 4, 2019, employees were performing maintenance on the 6-Place Press 12 in Department 13, to change-out/replace a Mac/hydraulic valve with an inadequate energy control procedure for Press 12, thereby exposing employees to injuries associated with exposure to hydraulic pressure. The hazardous energy control procedure in place did not have a specific procedural step for isolating the hydraulic pressure associated with the hydraulic pressure lines on Press 12 when performing a Mac/hydraulic valve change-out. b. On or about November 26, 2019, and at times prior, employees were performing service and maintenance related job tasks on multiple 6-place hydraulic presses in Department 13. The hazardous energy control procedure in place did not have specific procedural steps for isolating the hydraulic pressure and bleeding the stored hydraulic pressure associated with the 6-Place Presses when performing the above tasks. Tenneco Inc. was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1160489, citation number 1, item number 1 and was affirmed as a final order on November 30, 2016 with respect to a workplace located at 1601 Highway 49B North, Paragould, AR 72450.
