29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a)The TJX Companies, Inc. dba T.J. Maxx at 1611 Eglin Street, Rapid City, SD 57711: An inpatient hospitalization that occurred on November 8, 2019, where onsite management was aware of the hospitalization on the day of the incident, was not reported until November 11, 2019. T.J. Maxx was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.39(a)(2), which was contained in OSHA inspection number 1044633, citation number 1, item number 1 and was affirmed as a final order on 6/1/2015, with respect to a workplace located at 221 Wade Road, Latham, NY 12110.
