29 CFR 1910.23(d)(2): (2) The minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder was not 7 inches: Boiler room- The fixed ladder used to access the storage area over the boiler room had 3.5 inches of clearance between the top ladder rung and the wall on which it was mounted. The ladder rungs below the top rung had 6 inches of clearance between the rungs and the wall on which it was mounted.
