29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On the production floor at 131 West Main St, Orange, MA, on and before October 24, 2019, employees did not utilize the existing energy control procedure to lock out all energy sources when setting up the Line 1 Amplas bag machine. On 10/24/2019, an employee received a finger amputation while reaching in to perform maintenance on the bag machine. Hi-De Liners, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.148(c)(4)(i), which was contained in OSHA inspection number, 1133572, citation number 1, item number 1a, and was affirmed as a final order on May 2, 2016, with respect to a workplace located at 131 West Main St, Orange, MA.
