29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about September 17, 2019, an oxygen explosion occurred involving an oxygen cylinder transfilling operation that resulted in an in-patient hospitalization of an aircraft service employee and the employer failed to ensure the hospitalization was reported within 24 hours. The employer reported the incident on October 7, 2019 via the OSHA website serious event reporting online form.
