29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On October 4th, 2019 for employees exposed to struck-by hazards from sloughing materials and excavating equipment while replacing a waterline up to eight feet deep in a trench with near vertical walls in the vicinity of 12th Avenue NW & Custer Drive in Mandan, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1335635, Citation 1, Item 1b, and was affirmed as a final order on November 21, 2018, with respect to a workplace located in the vicinity of 503 North Fourth Street in Bismarck, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1317076, Citation 1, Items 1a, and was affirmed as a final order on September 11th, 2018, with respect to a workplace located near the intersection of North Washington Street & Interstate Avenue in Bismarck, North Dakota.
