29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about September 10, 2019, a temporary employee sustained a fingertip amputation to the right index finger while operating a Micron America MC-20 Pneumatic Attaching Machine (Grommet Machine) and the employer did not report the amputation as a result of a work-related injury within twenty-four (24) hours.
