29 CFR 1926.1210(b): The entry supervisor failed to verify, by checking that the appropriate entries have been made on the permit, that all tests specified by the permit have been conducted and that all procedures and equipment specified by the permit are in place before endorsing the permit and allowing entry to begin: a) On or about 9/20/2019, 304 Front Street, Binghamton, NY: The entry supervisor allowed an employee to enter a permit required confined space which had previously held a flammable liquid prior to verifying all of the permit items were filled out, all of the pre entry tests were conducted, and all of the equipment specified on the permit was in place. Abatement verification is required for this item.
