29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At the USA Quartz facility: On or about July 11, 2019, an employee was hospitalized as a result of a workplace injury and the employer did not make a report to OSHA, within 24 hours of knowing that the employee had been hospitalized.
