29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to notify OSHA of work-related fatalities within eight hours. This condition most recently occurred on or about, August 31, 2019 at 3534 State Highway JJ, Moberly, Missouri 65270, when a workplace fatality occurred. OSHA was notified by an employer representative of the employee fatality at approximately 3:15 p.m. on September 03, 2019.
