29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about September 4, 2019, at the above addressed jobsite, employees were conducting water main installation operations in a trench measured at seven (7) feet six (6) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
