29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): (a) Thirteenth Floor Entertainment Group, LLC DBA 13th Floor Haunted House at 3400 E 52nd Ave Denver, CO 80216: On and before August 26, 2019, the employer did not verify through a written certification that a workplace personal protective equipment (PPE) hazard assessment had been performed. Employees were exposed to hazards such as chemicals, airborne dust, flying particles, and laceration hazards.
