29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: a. On or about 08/01/2019, at the paver manufacturing and palletizing area catwalk bridge over pallet driver 1, employees were exposed to a fall hazard, in that, two stairways providing access to the catwalk had handrails at 28 inches high and no intermediate rails. b. On or about 08/01/2019, at the paver manufacturing and palletizing area control room, employees were exposed to a fall hazard, in that, a 4 step stairway providing access to the control room was not provided with midrails as required. c. On or about 08/01/2019, at the paver manufacturing and palletizing area control room, employees were exposed to a fall hazard, in that, a 4 step stairway providing access to the control room was not provided with a stair railing system at the open side.
