29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about June 24, 2019 for an employee whose arm was crushed by a hydraulic die ram while he was cleaning hydraulic fluid from the die table of a Buhler Polymatik Pasta Extrusion Press (Line 4) at the facility located at 1801 North 36th Street in Grand Forks, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1123327, Citation 1, Item 1a, and was affirmed as a final order on March 21, 2016 with respect to a workplace located at 1801 North 36th Street in Grand Forks, North Dakota.
