29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: The employer does not protect each employee in its trench excavation by requiring the employee to wear protective helmet when working in a trench excavation where there are possible dangers of head injury from impact or from falling objects. a) This violation was most recently observed on or about July 24, 2019, at 60 Brentwood Dr., Glencoe, Illinois. An employee working in a trench excavation was not wearing protective helmet where there were possible dangers of head injury from impact or falling objects. Goose Lake Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1317957, citation number 3, item number 1 and was affirmed as a final order on November 19, 2018, with respect to a workplace located at 6001 Capulina Ave., Morton Grove, Illinois 60053. Goose Lake Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1189653, citation number 1, item number 2 and was affirmed as a final order on December 13, 2016, with respect to a workplace located at 374 Lee St., Des Plaines, Illinois 60016. Goose Lake Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1162229, citation number 1, item number 2 and was affirmed as a final order on August 15, 2016, with respect to a workplace located at 2515 W. Lawrence Ave., Chicago, Illinois 60606. To abate this hazard in the future, the employer must ensure that each employee is provided with a hard hat while working in a trench excavation where there are possible dangers of head injury from impact or falling objects. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect each employee from head injury while working in a trench excavation.
