29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about February 25, the employer failed to report a partial fingertip amputation that was the result of an employee whose finger came into contact with the large saw blade on the Cosen band saw (Saw #01)hat was in operation. The employee was trying to take a measurement of the graphite material that was already inside the machine, which was running.
