29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about May 17, 2019 the employer failed to ensure that the energy isolating devices of the Four-Head (350R2) blow mold rotogranulator were operatied in a manner to isolate the hazardous energies of the equipment prior to performing maintenance. Hazardous energies include mechanical and electrical. Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296891, citation number 1, item number 1 and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706.
