29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: a) On or about June 10, 2019 and at times prior, two drainage holes located at the production area were approximately 5 inches in diameter and the covers were approximately an inch deep below the floor surface. Employees were exposed to tripping hazards from stepping into the holes.
