Location: 31-37 Pearl Street Port Chester, NY 10573; Bread Production Department a) On or about June 3rd, 2019, employees were exposed to caught-in hazards due to lack of guards on the sprockets on the bakery machinery - Winkler FORMSTATION 49070 1997 had an opening of approximately 9 in x 21 in. b) On or about June 3rd, 2019, employees were exposed to caught-in hazards due to lack of guards on the sprockets on the bakery machinery - Winkler FORMSTATION 52020 2000 had an opening of approximately 5 ft x 3 ft. c) On or about June 3rd, 2019, employees were exposed to caught-in hazards due to lack of guards on the sprockets on the bakery machinery - Winkler SPREIZBAND 49071 1997 had two openings of approximately 3 ft x 3 ft and 4 ft x 3 ft.
