29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to provide timely report of an employee fatality. On or about 5/25/2019, at 3863 McDonald Avenue, Saint Louis, MO 63116, an employee who was engaged in tree trimming and removal services was fatally injured and pronounced dead on 05/25/2019. The employer did not report the death to Occupational Safety & Health Administration (OSHA) U.S. Department of Labor.
