29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: The employer is failing to notify OSHA of work-related hospitalization within twenty-four hours. This condition most recently occurred on or about 05/28/2019 at 3003 S. Broadway St. Wichita, Kansas 67216 when an employee was hospitalized from a work-related incident at approximately 11:30 a.m. OSHA learned of the incident at approximately 3:20 p.m. on 05/30/19.
