29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): WORKSHOP: The employer had not verified through a written certification that the Workplace Hazard Assessment for personal protective equipment (PPE) for the employees performing tasks, such as (but not limited to the tasks listed below) had been performed as complete: Changing light bulbs and removing cob webs using the Toyota No. 6 powered industrial truck (PIT) and changing propane gas fuel tanks of the PIT.
