On or about 4/30/2019, on the roof of building 645 at Robins AFB, GA: The employer failed to report to OSHA a work-related hospitalization within 24 hours of such knowledge and did not contact OSHA of the following injury: On or about 4/24/2019, Amber Bentley, a roofer was hospitalized with a leg injury. OSHA became aware of this employees hospitalized injuries on 4/29/2019.
