29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: (a) S1 Parkinson Winder - On or about April 11, 2019 the employer did not provide stairs that had 5 treads and 5 risers with a mid-rail. The employees were exposed to a fall hazard.
