29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: On or about April 2, 2019 the employer did not meet the reporting requirement for reporting a work-related fatality to OSHA within eight hours from the time of the incident.
