29 CFR 1910.264(d)(2)(i)(a): The safeguard, safety appliance, or device attached to, or forming an integral part of any machinery was removed or made ineffective except for the purpose of making immediate repairs or adjustments: Work site was located at 6900 Roustein Avenue, Unit 1, North Bergen, NJ 07047 a) The lock mechanisms of washing machines used for laundering did not function properly with the locks bypassed using a knife allowing doors to be opened mid cycle. On or about 03/19/2019 an employee reached into a washing machine mid cycle resulting in an amputation to the worker's right hand. Violation occurred on or about March 19, 2019.
