29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Drilling site: On or about February 27, 2019, the employer did not ensure Big E Rig #6 was grounded as it was powered by 480v of power provided by onsite generators. This condition exposed employees to electrical shock hazards. Eastham Enterprises Inc. was previously cited for a violation of this occupational safety and health standard (1910.304 G05) , which was contained in OSHA inspection number 987727, citation number 1, item number 4 and was affirmed as a final order on 12/24/2014, with respect to a workplace located at Wolfbone (Trend Area) Field, Black Jack 16 Lease, Well #8H.
