29 CFR 1904.29(b)(3): Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. On 2/13/19, at the Blairsville facility, the employer did not record an injury on the OSHA 300 log within 7 days of the event. An employee received an amputation on 1/22/19 and as of 2/13/19 had not recorded the injury on to the 300 log for the facility.
