29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 02/11/19 on Line #1; employees were exposed to caught between hazards in the point of operation on the pouch filler machine, such as but not limited to, emptying the overs buckets below the pouch filler, removing misaligned bags on the filler sleeve index rack , when the interlocked doors access doors on the south and north side of the machine were missing and the interlocked access door on the west side of the machine was open while the machine was running. ABATEMENT DOCUMENTATION REQUIRED
