29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about 01/24/2019 at approximately 8:30AM, an employee suffered an amputation to the middle and index fingers of his right hand. The incident was not reported to OSHA until 01/28/2019 at approximately 1:30PM.
