29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Todd Schroyer Builder, LLC, on or about January 14, 2019, at the address of 2054 Driftwood Lane in Oregon, OH: the employer did not report to the local OSHA office of an employee hospitalization within twenty-four hours. An employee was hospitalized with injuries on January 14, 2019 after the employee fell approximately 20 feet from a scaffold. The Toledo area office was notified of the hospitalization on January 17, 2019.
