29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about January 9, 2019 the employer failed to make a notification to OSHA after they were made aware of an employee hospitalization. The employer was notified by a family friend of an employee that, as a result of a workplace injury that had occurred on January 5, 2019, the employee was now hospitalized in serious condition. The work-related incident was not documented, investigated or reported by the employer until after the employee had passed away on January 11, 2019.
