29 CFR 1904.29(a): A log of all work-related injuries and illnesses (OSHA Form 300), and/or the summary of work-related injuries and illnesses (OSHA Form 300-A), and/or the injury and illness incident report (OSHA Form 301) or equivalent forms were not kept by the establishment: (a) On or about Janaury 18, 2019, the employer failed to maintain and record an injury on the OSHA 300 and 301 forms when an employee suffered chemical burns while preparing to spray a building surface to clean mildew.
