29 CFR 1910.119(d)(3)(i)(B): The employer did not ensure the Process Safety Information (PSI) were complete and accurate including but not limited to the Piping and Instrument Diagrams (P&IDs). This violation was most recently observed on or about January 8, 2019, in the refrigeration engine room ER-2, when the employer failed to ensure the PSI were complete and accurate including but not limited to the P&ID drawing R3.3. Discrepancies were: a - Missing the pressure indicators on the pumps discharge lines from pressure vessels Low Temp Recirculator and High Temp Recirculator. b- Multiple valves were unidentified on the drawings.
