29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: The employer failed to report to OSHA an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. This was most recently documented at 1600 N. Harrison St. O'Neill, Nebraska on or about January 14, 2019. An employee working for the company slipped on ice on the sidewalk and was hospitalized on December 31, 2018.
