29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees by ensuring the use of a ladder or other safe means of egress. This was most recently documented on or about January 11, 2019 at or near Stable Road, Saint Louis, MO 63110, employees were replacing a concrete storm water drain pipe in an excavation measuring approximately 7.5 feet deep X 29 feet long X 6 feet wide. A steep earthen ramp measuring an incline of approximately 50-60 degrees was used as access and egress, the path of egress was blocked by the Caterpillar 326 F Excavator bucket. The employees were working on the East side of the trench 25 feet from a safe means of egress.
