OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a) On or about January 5, 2019, at the Houston Refining LP facility at 12000 Lawndale St. Houston TX; the employer failed to follow specific procedural steps for installing a selected Isolation Test Plug, exposing employees to fire and possible explosion hazards while welding was conducted on a pipe that contained residual crude oil.
