29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) Formosa Plastics Corporation, 780 Schoolhouse Road, Delaware City, DE 19706 - On or about December 11, 2018, the employer failed to report the in-patient hospitalization of an employee within twenty-four (24) hours of the hospitalization. A report of this incident was not received by OSHA until December 17, 2018.
