29 CFR 1904.29(a): A Log of Work-Related Injuries and Illnesses (OSHA FORM 300), and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: a. On or about December 11, 2018, the employer failed to keep OSHA logs, summaries and injury and illness reports for 2016 through 2018. Rubens Spray Insulation, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(2), which was contained in OSHA inspection number 1097399, citation number 2, item number 1 and was affirmed as a final order on January 8, 2016, with respect to a workplace located at 2253 W. Huron Street, Chicago, IL 60612.
