29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) At the logging site - On or about November 28, 2018, employees were exposed to struck-by hazards. A Personal Protective Equipment assessment of the logging operation was not performed to determine if hazards were present which required the use of PPE, such as, but not limited to high visibility clothing and hard hats.
