29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: a) On or about November 19, 2018, at the knife area; the employer did not remove damaged extension cords from service. The extension cords, with damaged outer insulation heat damage to wiring, were used in this condition by employees to operate electrical hand tools.
