29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout and opening process equipment or piping. (These safe work practices shall apply to employees and contractor employees.) a) Delaware City Refining Company, 4550 Wrangle Hill Road, Delaware City, Delaware, Unit 22 Coker (Department), D2A Fluid Coker Reactor, Cuff Feed Nozzle 9th Deck South Side- On or about October 30, 2018 the employer failed to implement a safe work practice for safely securing a cuff feed nozzle for maintenance and returning it to service. The safe work practice developed for opening process equipment to remove a cuff feed nozzle for cleaning did not include steps for isolating energy, locking out, at the reactor block valve. Operators and contracted pipefitters were exposed to hot coke, some sustaining severe burns that required hospitalization. The Delaware City Refining Company, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.119(f)(1)(i)(C), which was contained in OSHA inspection number 1087168, citation number 1, item number 2, and was affirmed as a final order on 7/24/2016, with respect to a workplace located at 4550 Wrangle Hill Road, Delaware City, DE 19706. The Delaware City Refining Company, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.119(f)(4), which was contained in OSHA inspection number 1108618, citation number 1, item number 1, and was affirmed as a final order on 11/17/2016, with respect to a workplace located at 4550 Wrangle Hill Road, Delaware City, DE 19706.
