29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as follows: On or about October 23, 2018, in the truck shop, an open stairway with 13 risers leading to the sawmill supervisors office was not provided with a stair rail system that included a midrail thereby exposing employees to a fall hazard.
