29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to notify OSHA of work-related fatalities within eight hours. This condition most recently occurred on or about 10/02/18 at the Well Watch, LLC facility located at 13937 Hwy 283, Ness City, Kansas 67560 when an employee died from a work-related incident at approximately 2:30 pm. OSHA learned of the incident at approximately 9:30 am on 10/03/18.
