29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by kick back of parts during sawing: An anti-kickback guard that was specifically designed for the saw line was not used on the AZON aluminum cold saw (serial MACH-KIT-000002), such as on September 20, 2018, when a kickback injury occurred. Previous incidents, including an injury, have occurred. Quaker Windows And Doors was previously cited for a violation of this Occupational Safety and health Standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection Number 1235694, Citation Number 1, Item 1, Issued on 7/27/2017, and became Final Order on 8-22-17.
